Assessment Description

Describe the nurse’s role and responsibility as health educator. What strategies, besides the use of learning styles, can a nurse educator consider when developing tailored individual care plans, or for educational programs in health promotion? When should behavioral objectives be utilized in a care plan or health promotion?

Being an educator is one of the most important tasks that nurses play in illness prevention and

health promotion. The majority of patient interaction time is spent by nurses, who also offer

proactive advice on vaccines, diet, medicine, and safety.

For all nurses, patient education is a big obligation. Between the time of admission and the

time of discharge, a patient should get education. The ability to instruct patients and enforce

instruction is something that nurses can do at any time. Patient evaluation is the first step in

being a health educator and a teacher. The need for patient assessment stems from the fact

that every patient has a unique learning style, educational background, set of values, and

worldview. Assessing for any learning obstacles is another duty of nurses. Culture, health

disparities, the environment, linguistic, literary, and physiological limitations are only a few

of these obstacles. In order to manage their health, avoid illness, and promote health, patients

need to be educated. As part of an interdisciplinary team, nurses should construct a patient-

An interdisciplinary team and a nurse educator may work together to create a personalized

care plan. To improve the effectiveness of their instruction, nurses must learn what matters to

their patients and what drives them. Each patient will experience this differently because they

all have various learning motivations. To demonstrate excellent teaching, nurses should use

the teach back technique. It is crucial for nurses to concentrate on a particular target group

that has similar values and objectives when creating educational programs for health

promotion. The degree of literacy and any other learning obstacles must be identified. It is

important to determine the literacy level and any other barriers to learning. Providing various

resources such as video, written, and audio material is essential for teaching and evaluating

the patient’s knowledge in teaching. (Smith & Zsohar, 2013).

When a patient is ready to learn and change, behavioral goals should be included in their care

plan. Before using the behavioral target, a nurse must assess the patient's readiness for change

and develop an objective appropriate for the patient's stage. In addition to being essential in

enhancing patients' health and wellbeing, nurses also play a critical role in patient education.

Describe a health promotion model used to initiate behavioral changes. How does this model help in teaching behavioral changes? What are some of the barriers that affect a patient’s ability to learn? How does a patient’s readiness to learn, or readiness to change, affect learning outcomes?

The health belief model-rural health promotion and disease prevention toolkit states to focus on the individuals health conditions, how the patient perceives their illness or disease, how the perceive the severity of their illness, barriers that affect the person or community, and confidence to succeed.

The Health promotion model makes four assumptions. One assumption individuals actively search to regulate their lifestyle behaviors, individuals actively participate in the environment and in turn change themselves and the environment, and the individual actively participates to change their behaviors. Health promotion models help patient’s have a healthier lifestyle. There are different factors that individuals and communities have for an unsafe lifestyle behaviors such as not being active, unhealthy daily eating, smoking, and stress. Simpson(2015) states some of the reasons people show these types of unhealthy behaviors is because low income, lack of access to safe places for exercise, healthy foods are not readily available to people, and culture and ethical differences.

Some of the barriers that may occur are that individuals do not want to make the lifestyle change, they do not have the resources, their attitude towards their illness/disease, they may be in denial, and they just aren’t ready to. Looking through this model, it reminded me of a diabetic patient that we gave education daily and the patient was not ready to make lifestyle changes. The patient then had a toe, and then another, and another, until it was a below the knee amputation due to being non-compliant. It was at that point the patient was ready to make the change to their lifestyle. It doesn’t have to get to that point if the correct resources and education is given to patients.

There are several models used to initiate behavioral changes. The following are examples:

  1. Socio-ecological Model: Addresses behavior change at multiple levels; consider inter-relationship between behavior and environment; account for multiple factors that influence behavior change process.
  2. Trans-theoretical Model (stages of change): accounts for readiness to make and sustain behavioral changes
  3. Health Belief Model: addresses readiness to act of health behaviors based on individual beliefs.

I’ll focus on the Trans-theoretical for this assignment. This model helps in the teaching of behavioral changes by focusing on five stages.

  1. Pre-contemplation: No intention to change withing the next 6 months; could be due to lack of knowledge or unsuccessful previous attempts at change.
  2. Contemplation: Considering change within 6 months; Ambivalence may keep individual from progressing.
  3. Preparation: Individual takes steps towards change within the next 30 days
  4. Action: Apparent behavior change for 6 months or less.
  5. Maintenance: Behavior change that has lasted over 6 months.

Barriers that affect a persons ability to learn can vary from intellectual, physical, mental, Social, or even the teachers personality. “The supreme value of a teacher is not in the regular performance of routine duties, but in his power to lead and to inspire his pupils through the influence of his moral personality and example; Strictly speaking, personality is made up of all the factors that make the individual what he is, the complex pattern of characteristics that distinguishes him from the others of his kind; Personality is the product of many integrating forces”(yourarticlelibrary.com).

When a patient is ready to learn/change outcomes are more apparent due to the motivation. The nurse should be able to distinguish between the changes of stages; however, at the same time be aware that progress may go forward or backwards. “While there are some similarities to the socioecological model, this model focuses on helping the individual to move through the stages toward a sustained behavior change”(extension.purdue.edu).

Topic 2: The Form And Function Of Family

What is a definition of family that encompasses the different family structures prevalent today? Discuss the importance of acknowledging nontraditional family structures. Explain how family systems theory can be used to better understand the interactions of a modern family (traditional or nontraditional).

When watching a video on study.com, it starts out with how different people live in a home, apartment,

duplex, and condo. They state that when we think of the people that live inside of these homes, it is

usually a family. The family consists of a mother, father, and their children. This type of family is

considered a Nuclear family. Study. com states in 1960, 73% lived in Nuclear homes. By 2015 only 46% were being raised

in a Nuclear household The video lists the different kinds of family structures. There is a single parent

family that is just one parent who cares for the children without the help of the biological father or

mother. Same sex families where it is two women in the home with the children, or two men in the home

with the children. Step families that the mother and father of the children separate or divorce and remarry.

Lastly, is the extended family, which is relatives of the children taking care of them in the home.

The importance of acknowledging non-traditional family structures is important because when we

acknowledge different family structures it helps the family feel like they belong, it gives the family a

sense that they have support from others around them, and it helps boost self-esteem and confidence for

that family. Another good point that was brought up when reading was that kids may look at other kids’

family structures differently and that parents should teach their children that there are other kinds of

family and not just a mom, dad, and children.

The family systems theory can be used to better understand the interactions of the modern traditional and

nontraditional. The Calgary Family Intervention Model(CFIM) helps nurses assess family strengths,

resources, problems, and illness. The nurse asks the family targeted questions that help assess structure,

development,and function between the family. CFIM helps develop a relationship with the nurse and the

family so the family feels comfortable answering the questions the nurse is asking. The CFIM asks about

specific family structures, health-related burdens, and experiences along with if the family has resources

and if there are problems within the family (Zimansky, Stasielowicz, Franke, Remmers, Friedel, &

Atzopodien 2020).

Why is the concept of family health important? Consider the various strategies for health promotion. How does a nurse determine which strategy would best enable the targeted individuals to gain more control over, and improve, their health?

Understanding the structure of families and family health enables the nurse to see beyond the individual patient. Viewing the family as a whole provides perspective to the nurse on how to best promote health (Green,2018). Nurses assess the health of the entire family to identify health problems and risk factors, help develop interventions to address health concerns, and implement the interventions to improve the health of the individual and family. Family nurses often work with patients through their whole life cycle (Petiprin,2021). The family unit is a vital component of health promotion. Nurses can guide families to the appropriate resources for their particular health concerns. Nurses can also provide health information and education to families to help support one another. Nurses can utilize the Calgary Family Assessment and Intervention Models(CFAM) for assessing families. CFAM helps to create a multidimensional view into a family’s life, using the categories structural, developmental, and functional. Interventions include areas focused on cognitive, affective, and behavioral patterns (Green, 2018). These interventions are selected to connect with and improve aspects of cognitive, affective and behavioral domain. Therapeutic communication should be used by the nurse to better understand each unique family. The nurse role in preventative health care is to utilize evidence-based research and recommendations to improve the health of patients. Nurses are the catalysts for healthier lifestyles through encouragement and teaching, helping patients to potentially receive preventative services such as counseling, screenings, and precautionary procedures or medications. Nurses can encourage families to engage in healthy lifestyles through education, mentorship, and leadership Nurses can assist families in navigating the health-care system. Nurses are consistently working to prevent illnesses such as heart disease, stroke, diabetes, and obstructive pulmonary disease; they do this through a variation of tactics that include education, risk factor prevention, and the monitoring of safety hazards either in the workplace, community, or home (Coloradotechedu). Through assessment and the nursing process, nurses can determine which strategies would be best for particular individuals and families.

Why is the concept of family health important?

The concept of family health is a very important part in maintaining and improving the health of the individual and community. The improvement in one unit will reflex on the other. The concept of family health is key for the attainment of goals of health promotion.

The concept of family health enables family members to know more about their genes which they share.

It enables family members to realize more about their traits such as curly hair as well as other abilities such as athletic abilities.

It enables family members to know about the level of risks to diseases such as asthma and cardiovascular diseases.

Today the medical community realizes that families play an essential role in safeguarding patients. Families are helping patients adhere to medical advice, by monitoring and encouraging them to exercise and listen to the instruction of health care professionals. This is because the family unit as a whole is a foundation for every person/ A partnership between a family and their healthcare provider is characterized as family-centered care.

The various strategies for health promotion

Creation of supportive environment

Equipping of individuals with personal skills

Strengthening of community action towards health

Reorientation of health services

Building of healthy public policies to ensure healthy living within different communities.

Coming up with programs such as smoking cessation programs which promote quitting of smoking habits

How nurses determine which strategy would best enable the targeted individuals to gain more control and improve their health

The selection of strategy will be based on the needs of the community. Strategies can be formed to also include the family for emotional and physical needs. With family support the individual can gain control over their well-being. Choosing strategies need effective planning and implementation. Before selecting a strategy consider the successful strategies used in other communities. Also, careful review of literature to identify the best strategy to address the problem then select a strategy to achieve the desired outcome. Afterwards, an evaluation of the selected strategy and replanning and lastly, development of timeline-oriented strategies.

For the targeted individual the following are strategies:

Health Communication is the use of communication strategies to intimate and influence the decisions of the individual to enhance health. It includes the verbal and written strategies and allows to make healthier choices. Health communication utilizes language and concepts based on culture, internet access, mass media etc to convey health related information. It can influence the community health in many ways like increasing the risk perception, reinforcing the positive behaviors, empowering health conditions.

Health Education: is one of the most important strategies for developing and implementing health promotion and disease prevention policies and programs. Health education can provide learning experiences for a target population on specific health topics

Health Policy, System and environmental Change: Policy Change is made by organizations, agencies, and stakeholders to improve the health. Examples are policies for smoke-free zones and public events, fat tax on junk foods.

·        Systems Change: is based on the change in the method of problems are solved. The systems change influences the purpose of the organization, function, and connections. It also acts by addressing organizational culture, policies, beliefs, relationships, and aims. Examples: adoption of the proven health promotion model

·        Environmental Change: aims at changing the economic, social, or physical surroundings or contexts which influence the health outcomes. Examples: building parks, greenways, and trails in the community

Continuous monitoring and development programs are essential for success. Nurses can develop individualized health care strategies through discussion about the health history of the family. This enables a patient to be more aware about genetic as well as environmental influences that can have a negative impact on their health hence being able to manage them.
