Ethical Issue
According to the VA Office of Rural Health, almost 2.7 million veterans live in rural or highly rural communities, where even basic healthcare may be hundreds of miles away (VA Office of Rural Health [ORH], 2016). In order to meet the unique need of these rural Veterans the VA initiated a robust initiative to increase access to health care. In 2016 the most promising of these remote initiatives include telephone-based delivery of care, connecting veterans with veteran eligible resources, community incentives for healthcare training, and telehealth/telemedicine programs (ORH, 2016). For this discussion I am going to focus on providing remote healthcare that eliminates the need for driving distances in light of the technological challenges to providing privacy and security protections. In using telehealth programs, the information exchange is not simply between the patient and the clinician, in that, many of these programs are maintained by a third party and this poses a security challenge for all concerned (Croll, 2010). In fact, the concern for security breach is often mitigated by the trust the patient has in the clinician and the clinician’s unawareness (Croll, 2010). Therefore, the ethical concern is the risk of security breach versus the health outcomes for the Veteran. Why are many health care organizations hesitant to adopt electronic health records (EHR) when they have the potential to improve quality, increase access, and reduce costs? Some organizations are concerned with the ethical and legal issues that may arise in daily operations. In 2010, legislators noticed health care’s reluctant transition into full EHR integration, and began to provide financial incentives to those organizations that not only adopt EHRs, but also use them meaningfully to improve quality of patient care. Usability and design challenges
Staggers (2010) states that usability has a strong, often direct relationship with clinical productivity, error rate, user fatigue, and user satisfaction which make for difficult adaptation. HIT usability is the thread that binds a product to specific users in a specific context in ore to achieve set goals with ‘effectiveness, efficiency and satisfaction’ (ANA, 2015). Nurses or end users are the ones that need to buy into the usability and clinical application design of new technology presented to them (Ball, Douglas, & Walker, 2011). The Technology Informing Education Reform (TIGER) summit highlighted the significance of both through identification, definition, and presentation of two pillars, informatics design, and technology, from and in which to develop their knowledge and beliefs. For usability to be effective, the goals must be met. Being able to affect and transform workflow positively, work with system developers to maximize clinical system development effectiveness with known processes and principles are the goals of which Ball et al. (2011) spoke. Implementing the case study’s new health information technology system
To implement an almost seamless transition from paper to the new HIT system, I would conduct a needs assessment, survey the facility to locate positions of possible placement of system, check workstation needs, the staff’s ability to learn and their receptiveness to change. Have systems in place in the facility prior to having experienced educators who are familiar with the system, present the HIT or have a train the trainer in-service.

You may either 1) be ecstatic about the needed change that this system brought, or 2) frustrated because you feel the system brings new hurdles into your already busy day. If you have ever experienced the latter option, the comment “I could have implemented a better system than this” may resonate strongly with you. Though research agrees that user input assists HIT designers, the development and implementation of new systems is not as straightforward as it may appear. HIT development teams find themselves in constant phases of trial and error as they strive to meet the needs of all users within the proposed development timeline. Failure to achieve promised usability benefits can lead to increased costs and job complications. Applying effective strategies to overcome usability challenges is essential.
